Taking control of your diabetes
Jump Start Diabetes Program
The Jump Start Diabetes Program is a free, educational program for patients with Type 2 diabetes. The program is a research project. The program intends to study diabetes understanding and the ability of patients to manage their diabetes. The research study is voluntary. No personal data will be collected. It is our goal that through this project we can learn how to present diabetes information that is easy to understand. There will be 3 short surveys that we would like you to take before and after completing the educational program. I ask that only adults 18 years of older with type 2 diabetes participate. Thank you for your participation.

What can I do the get control of my blood sugars?
The Association of Diabetes Care and Education Specialists outline 7 ways to help people improve their ability to manage their diabetes. These area include improving mental well-being, healthy dietary habits, exercise, monitoring your diabetes, taking medications, how to reduce risks and how to solve problems related to your diabetes. Please click on the box below to access the information. You may exit each learning page by clicking on “Jumpstart Diabetes”.
